CCUSD ATHLETIC CLEARANCE PROCESS

Culver City High School & Middle School have streamlined their athletic clearance process.  CCHS & CCMS athletes must be cleared through www.athleticclearance.com.  PARENTS create an account for this website to create clearances for each of their children.  (Please do not use student’s CCUSD email addresses for account set up)

If you do not have a copy of a physical exam taken within the last year, physical exam forms are to be printed from the site or from the school athletic website or picked up at the school and filled out by a physician.  Upload physical exam forms to www.athleticclearance.com.  

Please do NOT submit hard copies to the front office or to coaches.  Forms must be uploaded to the "Athletic Clearance" website.  Summitting hard copies of physicals does not clear a student.  Once your child is cleared parents will receive a confirmation email from Athletic Clearance.
