Yellow sheet for Defenses permission

Student received a yellow sheet of requirements for defenses (or retakes) that are being held the 2nd week after the holidays. Need to be completed before 1/15 which is the end of the semester. Please read carefully, practice over the break and turn in signed portion to me BEFORE the day of your retest.
