Culver City High School & Middle School has streamlined their clearance process.  Athletes must be cleared through www.athleticclearance.com.  PARENTS create an account for this website to create clearances for all of their children. (Do not use student’s CCUSD email addresses). Physical exam forms are to be uploaded to www.athleticclearance.com. (Please do NOT submit hard copies of your child's physical to the front office or to coaches) Should you have any questions, please contact Angelo Sablan via email at [email protected]

 

Once your child is cleared parents will receive a confirmation email from the Athletic office.  

 

In addition to being athletically cleared, students must now present an ATHLETIC CLEARANCE CARD to their coach on the first day of tryouts.  This will help insure that all students are cleared and ready to go on the first day. Students can pick up their card with Mr. Sablan in the PE office during lunch or after school, or Ms. Mathiesen & Ms. Denicia in the front office.  Students must do this for every sport they participate in. 

 

 

All donations for CCMS sports programs are to be entered here: https://www.pantherpartners.com/athletics.html. We request a $100 donation per athlete per sport so that we may provide quality coaches, uniforms, equipment, league fees, certificates to each player at the end of the season,  and so that we can continue to offer a wide and deep athletic program. If you are unable to contribute the full amount please donate an amount that is affordable. You may also choose not to donate by clicking on the appropriate button.

Sports eligibility Requirements
 
1. Students must have a physical before they can participate in any school sports including practices and games.  No student will be able to participate without the approval of a physician. NO EXCEPTIONS.  If you are unable to obtain a physical from your physician call the Health Center to schedule an appointment at (310) 392-8636.
2. Maintain a 2.0 GPA (cannot have 2 or more N's and no more than 1U) or you will be ineligible to participate in the program.
3. A student may be declared ineligible for disciplinary reasons by administrative action.
