Culver City High School & Middle School has streamlined their clearance process. 

 

ALL students that participate in sports must be athletically cleared -

NO EXCEPTIONS  

 

Athletes must be cleared through www.athleticclearance.com

 

  1. PARENTS create an account for this website to create clearances for all of their children. (Do not use student’s CCUSD email addresses)

 

  1. CCUSD Physical exam forms are to be uploaded (Please do NOT submit hard copies of your child's physical to the front office or to coaches) Physical Forms

 

 

  1. Once you get email confirming your student is cleared have them go to the office ask for Ms. Denicia to get their clearance card. (Before school, during nutrition/lunch, or afterschool)

 

  1. Present the ATHLETIC CLEARANCE CARD to their coach on the first day of tryouts. 

 

Students must get a clearance card for every sport they participate in. 

 

DONATIONS:

All donations for CCMS sports programs are to be entered here: https://www.pantherpartners.com/athletics.html

We request a $100 donation per athlete per sport so that we may provide quality coaches, uniforms, equipment, league fees, and so that we can continue to offer a wide and deep athletic program.

Sports eligibility Requirements:

1. Students must have a physical before they can participate in any school sports including practices and games.  No student will be able to participate without the approval of a physician. NO EXCEPTIONS
 
2. Maintain a 2.0 GPA (cannot have 2 or more N's and no more than 1U)or you will be ineligible to participate in the program.
 
3. A student may be declared ineligible for disciplinary reasons by administrative action.
