IMPORTANT INFORMATION

Culver City High School & Middle School have streamlined their clearance process. CCHS & CCMS Athletes must be cleared through www.athleticclearance.com. PARENTS create an account for this website to create clearances for all of their children. (Please do not use student’s CCUSD email addresses) Physical exam forms are to be uploaded to www.athleticclearance.com. Students MUST have a physical before participating in any after school sports activity. No student will be allowed to participate without the approval of a physician. No exceptions! If you are unable to obtain a physical exam from your physician, call the Health Center at (310)392-8636 to schedule an appointment (just let them know that you want it at the CCMS campus location).

In addition to being athletically cleared, students must provide an Athletic Clearance Card during tryouts in order to participate. Athletic clearance cards are issued by the following CCMS staff members:

All donations for CCMS sports programs are to be entered here: Athletic https://www.pantherpartners.com/athletics.html. We request a $100 donation per athlete per sport so that we may provide quality coaches, uniforms, equipment, league fees, certificates for each player at the end of the season, and so that we can continue to offer a wide and deep athletic program. If you are unable to contribute the full amount please donate an amount that is affordable. You may also choose not to donate by clicking on the appropriate button.
