Please print and submit the acknowledgement form AND the application, including any required attachments, to the CCMS Office (Attention to Tiffany Mathiesen) to be considered for Independent Study PE. Complete applications will be reviewed by a committee to determine if all criteria for Independent PE is met. If you need a printed copy of the forms, please visit the CCMS office to request it to be printed.
